The care model

Care that connects
with everyday life.

Rare disease needs extend across clinical care, family life and wider support. The Centre brings clinical teams, charities, patients, families and support organisations together through a common environment, extending into education, welfare and everyday life.

The person at the centre

People's information, experience and priorities provide a common focus for the relationships around them.

02

Support across boundaries

Professional resources and wider participation can sit within the same service environment, with appropriate roles and access.

03

Room to evolve

NHS Wales can develop the ecosystem as needs change, adding services, resources and participating organisations.

A Welsh model with wider potential

Wales on the
global stage.

The Centre is helping position Wales as a leader in person-centred rare disease care. Its model is attracting international interest as a way to connect services around people.

Its shared foundations offer a potential blueprint for chronic care and other complex, long-term conditions, shaped by each population and setting.

“The WDRCC demonstrates how people with rare diseases can be connected to the right support, at the right time, through a single trusted environment.”

Carecircle's role

Shared infrastructure.
A service shaped
around people.

Carecircle powers the Centre under a five-year NHS Wales agreement. Its configurable infrastructure brings together information, relationships, participation and governed workflows.

  • Person-owned information. Consent and permissions govern how information is shared and used.
  • Multilingual participation. Supported content and interactions help people participate in the language that works for them.
  • Reusable capabilities. The same foundations can be configured for further services, populations and use cases.
